We welcome you to complete the registration process prior to your initial appointment. Simply download then print the registration forms. Fill out the required information and bring them with you to your initial appointment. We strongly urge that you keep a copy of the registration forms. Please let us know if you need us to provide you with a copy.
We understand that some of these forms may be challenging, time-consuming, and in places redundant. We want you to know that the more information we have, the better we will be able to assist you and your family. If at any time in this process you have any questions, please contact us.
If you cannot make your appointment, we require 24 hours’ notice in non-emergency situations or you will be charged for the missed appointment. Please feel free to call with any questions you may have prior to this appointment. Please keep us informed of any telephone or address changes so that we will be able to get in touch with you.
